Online Registration

Each senior citizen is invited to register for the NCSC database build-up campaign.  Your honest and sincere support will help the NCSC develop an excellent senior citizen information system in the country, to be used as the basis for plans, programs, projects, activities, and events that will help improve the lives of older Filipino persons.

Step 1
FILL UP THE ONLINE FORM

Fill up the online form completely, correctly, and honestly.  Affix your recent photo and signature directly in the form.  You can also attach the image copy of the original SCDF or any valid government issued ID that you have in your possession for validation purposes.

Step 2
SUBMIT THE ONLINE FORM

After you have completely filled up the online form and successfully affixing your photo, signature, and image copy of the original data form or govenrment issued ID, click the 'Submit' button at the bottom of the page.

Step 3
CHECK REGISTRATION

When your submission is successful, you will be prompted with a confirmation page and will be provided with a Registration Reference Number known as RRN.  Please keep this number and the instruction that comes with it.  You can now check the status of your registration anytime at the verification page.

Step 4
APPROVAL OF ONLINE FORM 

Your registration to the database build-up campaign of the NCSC is now confirmed.   Please check out our official announcement of the approval provisions regarding the authentication of your data.  You will be notified via the contact information you have provided. 

Step 5
LOGIN TO YOUR ACCOUNT

When your registration data has been verified through our authentication process, your registration will be upgraded to verified registrant.  When you become a verified registrant, you will be provided with an online account where your can login and check for all activities concerning your registration.

What is a Senior Citizen's Data Form or SCDF?

The Senior Citizen Data Form known as the SCDF is the official registration form used by the National Commission of Senior Citizens to be filled up by the registering senior citizen.  It is composed of four (4) sections to contain information of the data subject's personal information, economic profile, health condition, and skill set identification.  It is a self-serving form that must be accomplished by the senior citizens themselves.  In case the registering senior is not capable of filling out the form, it is recommended to have a close family member or somebody known to him/her assist in the accomplishment of the form.

What does it mean when you register with NCSC?

The National Commission of Senior Citizens or NCSC has been mandated to alleviate the living condition difficulties of all the seniors citizens of the country and provide programs that will improve, develop, and protect the well-being of each.  By this, all senior citizens are invited to register and be counted to join the community, and by registering, one will be considered as recipient of the programs and activities the commission shall provide to perform its mandate among the seniors. 

What is your responsibility when you register?

By registering, it is your responsibility to provide an accurate, complete, and updated record of your personal profile, family composition, educational attainment, enumeration of skills, and medical condition.  By providing such information, NCSC will be able to design its programs and activities personally for you and thus, providing you assistance suitable for your specific need and condition.
 

What is the obligation of NCSC when you register?

By registering, NCSC acquires the information dataset to be used in the validation of each registrant.  It is the obligation of NCSC to protect and secure the information obtained from the registration form and commits to use the information solely for the purpose of performing its mandate.
